For instance: if you plowed at 8am and salted at 8pm that is two different services/visits (two entries on this form) OR if you did a full salt at two sites that is two different services/vists (two entries on this form) OR you plowed/ssw/psw/salt at 8am that is a single service/visit (one entry on this form).
